We work hard at PREPARE and UCSF to make sure all of your information stays private and safe. All online communications via PREPARE are fully confidential.
PREPARE collects personal information for use on the website. If you decide to register with PREPARE and sign in, then the PREPARE website will know and keep the information you entered in the registration process. This information could only be a username and password, but if you decide to enter in your name or an email address, then PREPARE would have access to that information too.
PREPARE will keep this information confidential and safe too. PREPARE uses information: to customize the content of the website, fulfill your requests, improve our services, contact you if needed, conduct research, and provide anonymous reporting (information without your name on it) for internal and external purposes.
The University of California and the PREPARE website does not rent, sell, or share personal information about you with other people or non-affiliated companies. UCSF only shares information to trusted partners who work on behalf of or with the University of California under confidentiality agreements to help us provide you with the PREPARE website, or as required by law.
We strictly limit access to any personal information to University of California employees who require that information to provide you with products or services, to answer your questions, and to improve PREPARE.
PREPARE, like many websites, uses Google Analytics to see how many people go to the website and how they use the website.
We also use Google’s Advertising Reporting Features to help us evaluate and optimize the way we promote PREPARE. Google uses cookies and can view your IP address, but PREPARE and UCSF does not send Google your name or other personally identifiable information.

OWNERSHIP: All right, title, and interest in and to the website known as PREPARE (“Licensed Material”) shall at all times remain with The Regents of the University of California (“The Regents”). User acknowledges that the Licensed Material is a copyrighted work and as such is protected by the copyright laws of the United States and by international treaties. User shall not copy, distribute, modify, or publicly display, nor permit any of its personnel to copy, distribute, modify, or publicly display the same for any purpose that is not specifically authorized under this Agreement.
PERSONAL USE LICENSE: The Licensed Material may be used by individuals (Users) for personal use, in which case The UC Regents grants the User a fully paid-up (i.e., no fees), nonexclusive, nontransferable, revocable right, without the right to sublicense, to reproduce, distribute, and publicly display the Licensed Material. All Users agree that they will not provide the Licensed Material to any other individual or organization not specifically authorized by this Agreement. The User may not modify or change the Licensed Material, other than to complete the Action Plan.
LICENSE FOR OTHER USES: All other uses, including those by companies, institutions, organizations, and agencies, require a separate license agreement from The Regents, except that (i) a link to PREPARE may be provided as a resource to individuals for their personal use, and (ii) the PREPARE written materials that are made available on the website, including the PREPARE pamphlet, the PREPARE advance directive, the PREPARE questions, and the PREPARE Toolkit, can be reproduced without any changes or modifications. Rebranding is not allowed unless there is a licensing agreement to do so.
PREPARE cannot be used by any companies, institutions, organizations, or agencies for clinical demonstration projects or research purposes, including studies, grants, quality improvement projects, or any form of publication without a separate license from the Regents of the University of California and the express permission of the author, Rebecca Sudore, MD (info@prepareforyourcare.org).
If a company, institution, organization, or agency would like to add their own branding and logos to PREPARE, modify PREPARE, or work with the PREPARE team to license specific content or use PREPARE in a research study, please contact the author, (info@prepareforyourcare.org).
INDEMNIFICATION and LIABILITY: The User agrees to indemnify, defend and hold harmless The Regents, its officers, agents, and employees from and against any and all claims, liabilities, demands, damages, losses, costs and expenses (including costs and reasonable attorneys’ fees) or claims for injury or damages that are caused by or result from User’s use of the Licensed Material, including but not limited to, any use of the Licensed Material that is not authorized under this Agreement.
USE OF NAME: Nothing in this Agreement shall be construed as conferring rights to use in advertising, publicity or otherwise any trademark, trade name, service mark or the name of "University of California" or "The Regents of the University of California" or any abbreviation thereof. However, the use of The Regents' name as associated with the copyright notice for the Licensed Material shall not be restricted.
User understands that THE REGENTS MAKES NO REPRESENTATIONS AND EXTENDS NO WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED. THERE ARE NO EXPRESS OR IMPLIED WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE, OR THAT THE USE OF THE AUTHORED WORK WILL NOT INFRINGE ANY PATENT, COPYRIGHT, TRADEMARK, OR OTHER RIGHTS. APPLICABLE LAW: This agreement shall be governed by and construed in accordance with the laws of the State of California.
